ATHLETIC INJURIES

All injuries incurred while participating in a CYO sport should immediately be reported to Athletic Director, Jay Snyder, at This email address is being protected from spambots. You need JavaScript enabled to view it.. All injuries incurred during a CYO sponsored event should have an incident report completed. For injuries requiring medical treatment and detailed information on how to file a medical claim for reimbursement, please click here.

STUDENT ACCIDENT CLAIMS

If a student is injured during a school-sponsored event, please complete a claim form. Archdiocesan student coverage is "excess" over personal insurance and only reimburses expenses not-covered by other expenses. Please follow the procedures on page two of the form and send in the proper documentation as soon as possible after the event.
